The main respiratory muscle — diaphragmatic — is partially or completely excluded from respiratory movements, especially on the side of the surgery. Besides that, patients after gastrointestinal surgeries are characterized by postural disorders. These patients have the typical appearance: the body is slightly leaned forward, the head and shoulders are lowered, and the belly is held up with hands to decrease the pain in the surgical area during movements. This posture obstructs the activity of the respiratory and cardiovascular systems. In case of surgeries remedial physical training can be carried out both in the pre-surgical (in case of planned surgeries) and post-surgical periods. Abrupt limitation of motor activity (hypokinesia) in the pre-surgical period during many days of examination of the hospitalized patient leads to reduction in the muscle tonus, force, lung capacity, as well as disturbances in hemodynamics. Based on the abovementioned the tasks of remedial physical training in the pre-surgical period can be determined: to increase the psychoemotional tonus; to improve the functional state of the cardiovascular and respiratory systems; to train thoracic breathing; to improve the activity of the gastrointestinal tract; to teach exercises of the early post-surgical period to patients. Contraindications to physical exercises: critical general condition; high temperature (38-39 °С); severe pain; possible haemorrhage. In the post-surgical period to achieve the restorative effect of physical exercises on the organism of the patient we use static and dynamic exercises for small and medium muscle groups of extremities. To improve the functional state of the gastrointestinal tract exercises for the muscles of the anterior abdominal wall and pelvic area are used. Great attention in the pre-surgical period is given to teaching skills and exercises to the patients which they will need after the surgery: activation of thoracic breathing; expectorating with fixation of the area of the future post-surgical scar and lower parts of the breast; lifting the hips resting on the elbows and shoulder blades; changing from lying to sitting, standing positions; rhythmic contractions of perineum muscles and straining of gluteus muscles. Exercises shall be done in initial lying, sitting, standing positions, 1 or 2 times a day, using an individual method.
